Upon registering for Drama Kids of the Capital Region, all parents are required to sign our policy and release of liability agreements. Students are NOT able to participate in Drama Kids unless these policies are agreed to, signed and dated. They are included here for your review: Payment PolicyOur developmental drama program is a SCHOOL YEAR PROGRAM. Our 28 week session is divided into equal payments per month for your convenience. Choose from monthly payments or receive discounts for paying once (annual 10% disc) or twice a year (semi-annual 5% disc). Select one of the payment options shown in the e-payment field. When you enter your credit/debit card information, your card will NOT automatically be processed. Within 2 business days, our office will process your registration form and charge your credit card for the payment option you select, at which time your space in the selected class will be reserved. ASSUMPTION OF RISK & REKEASE OF LIABILITYAs the legal parent or guardian, I release and hold harmless Drama Kids International, Inc. and its franchise owners, employees, The Latham Center for the Arts, Christ's Church, First United Methodist Church of Delmar, King of Kings Lutheran Church, Good Shepherd Lutheran Church & all other affiliated class site premises, from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Drama Kids International, Inc. its franchise owners, and employees in route to or from any of said premises. This waiver of liability expressly includes internet transmission to and from, or in connection with, said program.
The undersigned gives permission to Drama Kids International, its owners and operators to send SMS text message alerts to the phone number provided. By providing your mobile phone number, you agree to receive informational text messages from Drama Kids. Message and data rates may apply. Message frequency varies. Drama Kids International reserves the right to photograph and videotape programs and participants for training, advertising, or any other lawful business purpose. Any photographs used will not identify students by name without written permission. Customer information is for internal use only and will not be sold to outside parties. In addition, the undersigned gives permission to Drama Kids International, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or conditions and/or declare the participant to be in good physical and mental health. Drama Kids International, Inc. and its employees, agents, and representatives, and its franchise owner affiliates and their employees, agents, and representatives (collectively ""The Company"") understand that when children gather, illness can occur; therefore, the Company cannot guarantee that you, your family members, or your child(ren) (""Participant (s) and Family Members"") will not come into contact with contagious illnesses. By clicking my acceptance on this electronic registration form, I hereby acknowledge the contagious nature of illnesses and voluntarily assume the risk that Participant(s) and Family Members may be exposed to illnesses by attending classes, camps, performances, and all other activities (the ""Events"") put on by the Company and that such exposure or infection may result in personal injury, illness, and death. I understand that the risk of becoming exposed to illnesses at an Event may result from the actions, omissions, or negligence of myself and others, including, but not limited to the Company and Participant(s) and Family Members. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any illness to Participant(s) and Family Members (including, but not limited to, personal injury, and death), as well as any damage, loss, claim, liability, or expense, of any kind, that Participant and Family Members may experience or incur in connection with my child(ren)'s attendance and participation in the Company Events (""Claims""). On my behalf, and on behalf of Participant(s) and Family Members, I hereby release, covenant not to sue, discharge, and hold harmless the Company from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Company, whether an illness occurs before, during, or after participation in any Company Event."
